# Prepare a policy change request.

A policy change is not effective until the authorized insurer or administrator confirms it. Start with the policy, named insured, requested change, requested date, and supporting documents.

## Identify the account

Provide the named insured, policy number, effective dates, and the person requesting the change.

## Describe the requested change

State exactly what should be added, removed, or revised and when the change is requested.

## Wait for confirmation

Do not treat a request, certificate, or email as proof that coverage changed.
